A survey of pediatric ICU nurses’ cognition,attitude and practice on daily sedation interruption

Objective To investigate the cognition,attitude and practice of pediatric ICU nurses on daily sedation interruption(DSI),and to provide references for clinical training. Methods Convenience sampling was used to conduct a questionnaire survey among 298 pediatric ICU nurses in 7 tertiary A children hospitals in China in July 2021. The general information questionnaire,questionnaires of knowledge,attitude,and behavior on DSI were self-designed. Results In terms of the cognition of 298 pediatric nurses on DSI,their knowledge score was 4(2,4) out of 10,and a total of 157 nurses(52.68%) said they did not know well or had never heard of DSI. In terms of attitude,their recognition score was 32(27,34) out of 40,and 217 nurses(72.82%) were willing to implement DSI,and 258 nurses(86.58%) wanted systematic training. In practice of nurses,221(74.16%) have not received relevant training,and 159 nurses(53.35%) never or rarely performed DSI. Conclusion Pediatric ICU nurses have a poor grasp of knowledge on DSI,while their recognition toward DSI is high. Their practice needs to be strengthened. Nursing managers should pay full attention to the training needs of pediatric ICU nurses and carry out systematic training on DSI basic knowledge and practical skills,so as to improve DSI practice level of nurses.

Qualitative research on the feeling and experience of medical staff on neonatal hospice care

Objective To understand the feelings and experiences of medical staff on neonatal hospice care,and to provide a reference for the development of neonatal hospice care. Methods Descriptive research methods were used to conduct semi-structured interviews with 13 neonatal clinicians and clinical nurses,nursing managers,medical social workers,hospice care doctors and nurses,and the data was analyzed using targeted content analysis. Results 5 themes were extracted,including the needs of the dying newborn,the needs of the family of the dying newborn,the communication between doctors and patients,the timing of intervention for hospice care,and the multidisciplinary team of hospice care. Conclusion Medical staff should pay attention to the needs of dying newborns for pain reduction,comfort care,nutritional support,sense of security,and the needs of families to accompany children,emotional support,information support,professional guidance,understanding and respect,when conducting the neonatal hospice care of newborn babies. Effective and good communication between doctors and patients should be promoted. It is needed to consider the timing of intervention and the child’s disease condition and family will. Multidisciplinary collaboration is a good model for the development of neonatal hospice care.

Study on the effect of hammock positioning in promoting the growth and development of premature infants

Objective To investigate the effects of hammock positioning on the growth and development indexes of premature infants,such as weight,body length and head circumference. Methods From January to August 2020,106 premature infants admitted to a tertiary children’s hospital in Hangzhou were selected as the research subjects. They were divided into an experimental group(n=53) and a control group(n=53) by random number table method. The control group was treated with conventional heating box. The experimental group was treated with hammock positioning in addition to conventional warm box care. The growth and development of infants in the 2 groups were compared. Results The weight gain of premature infants in the experimental group at the 2nd to 4th weeks,the 24 - hour sleep duration and the discharge weight were higher than those of the control group;the number of days required to restore birth weight and the length of hospital stay are lower than those of the control group,and the difference was statistically significant(P<0.05). After using hammock therapy,the heart rate and respiratory rate of the experimental group decreased,and the oxygen saturation increased,and the difference was statistically significant(P<0.05). Conclusion Hammock positioning plays a positive role in promoting the average weight gain per week,reducing the number of days to regain birth weight,increasing the duration of 24 hours sleep,stabilizing vital signs of premature infants and reducing the length of hospital stay,which can effectively promote the growth and development of premature infants.

Construction of neonatal hospice care service system

Objective To construct the hospice care service system for neonates for clinical practice guidance. Methods Based on humanistic nursing theory and comfortable nursing theory,a consultation questionnaire was developed via literature review and qualitative research during September to November 2020,and was distributed to 25 experts from 7 provinces and cities for Delphi expert consultation and neonatal hospice care service system construction. Results The questionnaire recovery rates of the 2 rounds of expert consultations were 100%;the expert authority coefficient was 0.852;Kendall’s concordance coefficients were 0.124~0.212 and 0.111~0.204 respectively (all P<0.001);the variation coefficients of the 2 rounds of item importance score were 0~0.34,0.07~0.24,and the variation coefficients of feasibility score were 0~0.39,0.08~0.24,respectively. The neonatal hospice care service system was formed with 4 first-level items,12 second-level items,and 38 third-level items,including multidisciplinary team cooperation,intervention timing,service recipients,service content. Conclusion The neonatal hospice care service system is scientific and feasible,which can provide certain guidance for the clinical practice of neonatal hospice care.
